Excell Home Care and Hospice in Oklahoma City seeks a licensed physical therapist to provide home health services under a physician's plan of care. The role involves evaluating patients, developing personalized care plans, and coordinating with families.

You will educate patients and caregivers, document progress, and collaborate with physicians and other professionals to support safe, effective treatment. Oklahoma licensure, CAPTE accreditation, CPR certification, and a valid driver’s license

Competenze

• Licensed to practice as a physical therapist in the state.
• Degree from CAPTE-accredited program.
• Two years of therapy experience; home health preferred.
• Current CPR certification.

Mansioni

• Develop and update individualized care plans with patient, family, and caregiver.
• Counsel patients, caregivers, and families on therapy.
• Educate patients and caregivers on treatments and home exercises.
• Prepare clinical notes and progress summaries.
• Communicate with the physician and other health professionals about the plan of care.
• Provide PT services per physician’s plan of care, including assessment and treatment.

Conoscenze

Licensed Physical Therapist

CAPTE-qualified degree

Two+ years experience

CPR certification
