Job summary

Elara Caring is seeking a dedicated Physical Therapist to provide in-home care, evaluate patients, and develop tailored treatment plans. You will monitor progress, adjust plans, and work closely with physicians and caregivers to ensure comprehensive, coordinated care.

The role requires documenting diligently, supervising assistants, and traveling between patient homes. APTA-approved program and active PT license are essential; at least one year of healthcare experience is expected.

Qualifications

• One year of experience in a healthcare facility is required.
• Ability to lift, push, pull, and move up to 50 pounds, including patient transfers and equipment.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide physical therapy services in the home setting.
• Monitor patient progress and adjust care plans as needed.
• Collaborate with physicians, caregivers, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation.
• Supervise therapy assistants and support staff as appropriate.
