Job summary

Elara Caring is seeking an Occupational Therapist to deliver in-home rehabilitation services across our branch network. You will assess patient needs, develop and execute a Plan of Care, and coordinate with physicians and the care team to ensure high-quality outcomes.

You will travel within the coverage area (up to 50%) and use adaptive equipment to support activities of daily living, safety, and functional mobility for diverse patients in their homes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance.
• Must be able and willing to travel 50%.
• Ability to frequently lift, push, pull, and support up to 50 pounds.

Responsibilities

• Evaluates the OT patient to determine rehabilitation needs and develops a care plan.
• Implements and revises the Plan of Care to ensure quality and continuity of care.
• Communicates findings to supervisor, physician, and care team.
• Initiates safety plans using patient, family, and community resources.
• Verifies the Plan of Care and provides care per physician orders and standards.
• Writes physician orders for additional visits and changes to the Plan of Care.
• Provides ADL training and adaptive equipment instruction to patients/families.
• Supervises COTA and other staff to ensure compliance with the care plan.
