Job summary

Olympia Assured Home Health, part of the Optum family, is seeking an Occupational Therapist to assess and address home health patients’ functional needs. You will deliver care per the plan of care, coordinate with the health care team, and document patient progress in a home setting.

Qualifications

• Current Occupational Therapy licensure in the state of Washington.
• Valid driver’s license and dependable vehicle or access to transportation.
• Ability to function in diverse home environments regardless of patient background.

Responsibilities

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Provide occupational therapy within scope and plan of care.
• Observe, record, and report patient progress and changes to supervising nurse/physician.
• Conduct patient assessments using functional ability tests.
