Job summary

Assured Home Health, a part of LHC Group and a member of the Optum family, is hiring an Occupational Therapist for home health care across Washington state. You will assess functional status, activities of daily living, and home safety, coordinating with physicians to develop plans of care.

Qualifications

• Current Occupational Therapy licensure in state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides OT services within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with other health care team members.
• Evaluates the patient's functional status and CMS documentation needs; consults with physician on the therapy plan of care.
• Observes, records, and reports patient responses to treatment and changes in condition to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional status using diagnostic and prognostic tests.
• Ability to work with clients in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
