AdvisaCare is seeking compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to deliver high-quality home health care to patients in Michigan. You will work independently, complete SOC visits, routine visits, and create care plans that drive great outcomes.

Qualifications

• Active RN or LPN license in Michigan.
• Strong assessment skills and clinical judgment, including experience conducting assessments and follow-up visits.
• Strong communication skills, including educating patients and families, explaining care plans, and handling Start of Care conversations.
• Home health experience preferred but not required.
• Experience collaborating with physicians and therapists.
• Reliable transportation and eligibility to work in the United States.
• Experience with Kinnser is a plus.

• Completing Start of Care (SOC) visits with confidence, compassion, and clinical excellence.
• Completing routine patient visits.
• Performing assessments that reflect the patient’s needs.
• Creating individualized care plans that set the tone for amazing outcomes.
• Educating patients and families so they feel empowered.
• Collaborating with a supportive team that values your judgment and expertise.
• Documenting in the EMR.
• Being the RN who makes patients say, “Wow… I feel better already”.
