Job summary

Yale New Haven Health is seeking a Home Health Aide to provide direct patient care in home settings under supervision of the RN Case Manager or Therapist. Responsibilities include personal care, documenting observations, and maintaining HIPAA compliance.

Qualified candidates have a CT-approved home health aide training or active student LPN/RN, valid CT driver's license, and the ability to lift up to 50 pounds; strong communication and flexibility are essential.

Qualifications

• High School education or GED preferred, but not necessary.
• Certified according to State of Connecticut Public Health Code regulations.
• CT driver’s license and clean driving record required.
• Excellent communication and interpersonal skills.

Responsibilities

• Provides competent and safe personal care per plan of care.
• Accommodates complex patients incl. bedbound, palliative and hospice needs.
• Uses proper body mechanics for transfers and ambulation.
• Assists with ROM exercises as directed and with exercise programs instructed by the nurse or therapist.
• Assists with meal preparation per prescribed diet.
• Maintains patient environment and cleans as needed.
• Reports untoward findings to RN Case Manager or therapist.
• Follows universal precautions and PPE usage.
• Documents care and observations as instructed.
• Ensures patient documents are signed and dated and handed in timely.
• Conforms to payroll standards.
• Complies with HIPAA.
• Complies with weekday and weekend commitments.
• Participates in staff meetings and in-service programs.
