Job summary

Pathwell Health is seeking a licensed vocational nurse (LVN) to provide skilled nursing care in home health settings. The role may include PRN shifts and requires a California LVN license and CPR certification. LPN experience is valued, and Spanish-language ability is a plus.

You will coordinate with physicians and the healthcare team, educate families on home health procedures, and diligently document vital signs and patient changes to ensure high-quality care.

Qualifications

• Associate degree or equivalent required; RN preferred if applicable.
• Bilingual Spanish is a plus.
• Must hold a California LVN license and CPR certification.
• Minimum one year of LPN experience.
• Driver’s license, car insurance, and car registration are required.

Responsibilities

• Provide skilled nursing care under supervision.
• Complete evaluation tasks, including vital signs and medication reviews.
• Coordinate with physicians, therapists, and other care team members.
• Assess and chart client conditions during visits.
• Maintain complete and accurate nursing documentation.
• Educate clients and families on home health care procedures.
• Monitor and report changes in patient conditions.
• Modify patient plans based on patient responses.
• Ensure safety in home/work environments.
