Job summary

CenterWell Home Health is seeking a Home Health LVN to provide skilled nursing care to patients in their homes under the direction of an RN. You will implement care plans and educate families to support recovery and independence.

Requirements include an active nursing license in the practicing state, a valid driver’s license with reliable transportation, CPR certification, and at least two years of LVN/LPN experience in a clinical setting. Home Health or Hospice experience preferred.

Qualifications

• Current nursing license in the practicing state.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under RN direction.
• Implement individualized care plans with patient and family.
• Educate patients and families on disease management and self-care.
• Maintain accurate records of patient care and outcomes.
• Coordinate with physicians and care team; assist with discharge planning.

Education

LPN/LVN license (active in state)
