Job summary

Pathwell Health seeks licensed practical nurses (LPNs) in Winchester, VA for full-time and PRN roles. You will assess patient health, implement care plans, maintain medical records, and support disease prevention and home health care education under supervision.

Ideal candidates have an associates degree (bachelor's preferred), Virginia LPN license, CPR certification, and at least one year of LPN experience.

Qualifications

• Associates degree required; Bachelor's Degree from school of nursing preferred.
• Current and Virginia Licensed Practical Nurse license.
• One year of LPN experience.
• CPR certification, Driver's License, Car Insurance, Car Registration.
• Strong interpersonal, written and communication skills.
• Bilingual Spanish preferred.

Responsibilities

• Provide skilled nursing care under the direct supervision of the Clinical Supervisor.
• Complete evaluation tasks, including vital signs and medication reviews.
• Coordinate with physicians, therapists and other individuals in the client’s care plan.
• Assess and chart observations of the client’s condition at each visit.
• Maintain complete and accurate documentation of nursing care.
• Work collaboratively with all members of the health care team.
• Provide education to clients and families on proper home health care procedures and strategies.
• Record patients' medical information and vital signs.
• Monitor, record, and report symptoms or changes in patients' conditions.
• Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
• Modify patient treatment plans as indicated by patients' responses and conditions.
• Monitor all aspects of patient care, including diet and physical activity.
• Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems.
• Perform other duties as assigned.
