Choice Health At Home is seeking a Home Health Field LVN/LPN (PRN) to provide compassionate nursing care in patients' homes in Westminster, CO. Under RN supervision, you will assess health status, administer medications, and perform wound care as part of individualized care plans designed to promote independence and recovery.

You will coordinate with physicians, therapists, and families, ensure infection control, and document care accurately to support quality outcomes and third-party payor

Qualifications

• Current LVN/LPN license.
• CPR certification.
• Valid Driver’s License.
• Valid Auto Insurance.
• HomeCare HomeBase experience preferred.

Responsabilités

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Promote Agency philosophy and administrative policies to ensure quality of care.
