Qualifications

• Current RN license required.

Responsibilities

• Perform comprehensive assessments across physical, functional, psychosocial, and environmental needs.
• Establish admission and re-certification criteria for home health care.
• Develop and document the plan of care to ensure quality and continuity.
• Educate patients and families on self-care, nutrition, and diet.
• Determine service scope and frequency based on acuity and needs.
• Initiate preventive and rehabilitative nursing procedures.
• Apply infection control concepts and standard precautions during care.
• Provide clinical directions to Home Health Aide and LPN/LVN.
• Prepare progress notes showing progress toward goals.
• Inform physicians of changes and outcomes while monitoring response.
• Monitor cases for third-party payor compliance.
• Communicate effectively with patients, families, and health professionals.
