Job summary

Devotion Hospice is seeking a Home Health Field LVN/LPN (PRN) in Westminster, CO. The role involves delivering high-quality nursing care in patients’ homes, assessing health status, administering medications, and wound care under RN supervision.

You will develop and implement individualized care plans, educate patients and families on treatment protocols, and collaborate with physicians and therapists to promote independence and well-being.

Qualifications

• Current LVN license in good standing.
• Valid driver's license.
• Valid auto insurance.
• CPR certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Under the direction of the RN, identify the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Plan and implement care in conjunction with the RN, in accordance with the POC.
• Apply infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Monitor assigned cases to ensure compliance with requirements of third party payor.
• Promote Agency philosophy and administrative policies to ensure quality of care.

About the Role:

The Home Health Field LVN/LPN plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective management of health conditions. This position requires the ability to assess patient health status, administer medications, and provide wound care while collaborating closely with physicians, therapists, and family members. The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being. This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance. Ultimately, the Home Health Field LVN/LPN contributes to enhancing patient outcomes by providing personalized, attentive care in a home setting, supporting recovery and chronic disease management outside of traditional clinical environments.

As a Home Health LVN , you will:
