3HC Home Health & Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to home health and hospice patients in North Carolina. The role requires adherence to individualized care plans and compassionate patient support, with strong collaboration with the RN and healthcare team.

The ideal candidate has an active NC LPN license, CPR certification, and reliable transportation. A minimum of two years in acute care is preferred.

Qualifications

• Licensed Practical Nurse licensed to practice in North Carolina.
• Minimum of two years’ nursing experience in acute care preferred.
• Maintains current CPR (BCLS) certification.
• Reliable with the ability to work with patients, families, and staff.
• Valid driver’s license and reliable transportation.

Responsabilités

• Administers skilled nursing care per the patient’s Plan of Care.
• Ensures patient comfort, supports family education, and monitors changes.
• Documents care in medical records and updates RN.
• Collaborates with the RN and physician on care plans.
• Rotates on-call weekends, after hours, and holidays as assigned.
• Maintains compliance with agency policies and quality standards.
