Qualifications

• Active LPN license in good standing in Pennsylvania.
• Graduate of an accredited school of nursing.
• At least one year of professional nursing experience preferred but not required.

Responsibilities

• Deliver professional nursing care to assigned patients.
• Assess physical and emotional status and home environment.
• Provide information on patient rights and procedures.
• Evaluate patient status and communicate with physicians.
• Perform procedures including medications and IV solutions.
• Supervise Home Health Aides.
• Assist patients with activities of daily living.
• Maintain patient records and reports.
• Participate in on-call rotation.
• Perform other duties as assigned.
