Lehigh Valley Companion Care Homes is seeking a compassionate Licensed Practical Nurse (LPN) to join our team on a PRN basis. You will provide skilled nursing care in patients' homes and community settings, prioritizing health and comfort.

The ideal candidate holds a PA LPN license, has at least 1 year of home care or long-term care experience, and can work independently on an as-needed schedule. Competitive pay and opportunities for professional development are offered.

Formación

• Current and valid LPN license in Pennsylvania (PA).
• At least 1 year of nursing experience in home care or similar settings preferred.
• Ability to work independently and manage patient care responsibilities.

Responsabilidades

• Provide skilled nursing care to patients in homes and community settings.
• Administer medications and monitor treatment plans as prescribed.
• Perform health assessments and document findings.
• Assist with personal care tasks as needed.
• Support mobility and physical therapy exercises to enhance well-being.
• Ensure patient safety and comfort through proper care and supervision.
• Coordinate with family and healthcare providers to ensure optimal care.
• Respond to medical emergencies and provide first aid as required.
• Document all care and maintain accurate patient records.
