Job summary

Direct Care Home Health Services is seeking a Licensed Practical Nurse to provide direct skilled client care under the supervision of a registered nurse in Washington, DC. You will follow the plan of care, perform treatments, and document care weekly to support positive client outcomes.

Requirements include a current DC LPN license, strong patient assessment and communication skills, and the ability to work independently in home settings.

Responsibilities

• Provide direct skilled client care under RN supervision.
• Follow care plan and administer treatments.
• Turn in weekly documentation for review.
• Conduct ongoing assessments of patient condition.
• Educate patient and family on diagnosis and care needs.
• Document medications and monitor for adverse reactions.
• Coordinate home health services with disciplines (PT, OT, Speech).
• Follow infection control and HIPAA guidelines.
