Job summary

Professional Healthcare Resources is seeking a Licensed Practical Nurse (LPN) to join our team in the Washington, DC area. You will supplement nursing care for home health and hospice patients under the direction of an RN Field Case Manager.

Responsibilities include carrying out treatments, maintaining records, and reporting status to Clinical and Case Managers. One year of clinical experience is required and home health experience is preferred for this role.

Qualifications

• Must be graduate of a practical nursing program and licensed in state of operation.
• Minimum of one year of clinical experience required.
• Home health or hospice experience strongly preferred.

Responsibilities

• Carry out treatments, teaching, observation and assessments as described in the Plan of Care and delegated by the case manager
• Maintain records and reports per regulatory requirements and company policies
• Provide daily report to Clinical Manager and Case Manager to review status of cases; participate in monthly clinical record review
• Participate in personal and professional growth and development
• Fulfill additional performance responsibilities as assigned and as necessary
