Consumer Directed Services Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home and community-based nursing care.

You will assess patients, administer medications and treatments, monitor vital signs, document care, and collaborate with RNs and physicians to follow care plans. Qualifikationen

• Current LPN licensure required.
• Strong clinical assessment and medication/treatment administration skills.
• Ability to monitor and report vital signs and health status changes.
• Experience supporting care plans with RNs and physicians.
• Clear communication and patient/family education skills.
• Proficiency with electronic health record documentation.
• Empathy, teamwork, and commitment to person-centered care.
