Prisma Health is seeking a Licensed Practical Nurse to administer skilled nursing care to patients in their homes under physician orders and the plan of care, with supervision by an RN. This position is bonus-eligible and involves home health duties in Tuomey.

The LPN will implement the care plan, assist with procedures, maintain records, educate patients and families, and participate in care conferences with the health team. Driving is required to visit patients.

Formación

• Completion of an LPN program recognized by the SC State Board of Nursing.
• Ability to exercise initiative and independent judgment.
• Acceptance of philosophy and goals of this agency.

Responsabilidades

• Implements the nursing care plan for each patient. Provides nursing services, treatments, and diagnostic and preventive procedures according to the plan of care as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Prepares equipment and materials for treatments. Assists the RN in performing specialized procedures.
• Observes aseptic technique as required. Observes signs and symptoms and reports to the physician and RN reactions to treatments.
• Maintains accurate and complete records of observations, treatments and care of patient.
• Teaches and counsels the patient and family regarding nursing care needs and self-care techniques.
• Participates in patient care conferences with other health team members.
• Submits changes in schedule to clinical manager daily.
• Attends staff meetings and in-services.
• Performs other duties as assigned.
