Job summary

All Ways Caring HomeCare is seeking a part-time Licensed Practical Nurse to provide direct and indirect care to clients in home settings across Carey and Hailey, ID. You will document actions, assist with medications, and support families under RN supervision.

Requires nursing degree, active state license, CPR, and ability to work independently or with a team in a home care environment. Flexible scheduling and on-call weekends may be required.

Qualifications

• Degree in Nursing from an accredited school.
• Experience administering medications.
• One or more years of nursing experience preferred.
• Home health care experience preferred.
• Current and unrestricted license in the state of the operation.
• CPR certification.
• Basic clinical skills.
• Professional appearance and demeanor.
• General knowledge of computers and the ability to learn industry software and documentation applications.
• Excellent oral, written, and interpersonal communication skills with all levels of personnel.
• Excellent organizational and time management skills with attention to detail.
• Ability to handle highly confidential information.
• Ability to work independently or in a team environment.
• Ability to develop critical thinking skills and solve problems.
• Ability to learn and complete educational goals.

Responsibilities

• Provides skilled nursing care per physician orders and Plan of care.
• Documents nursing actions and client status; communicates with family and physicians.
• Documents progress and outcomes for goals and informs physician, case manager, and families of changes.
• Medications administration and management as needed.
• Provides health teaching and assists client in learning self-care techniques and safety measures.
• Conducts nursing assessments as needed and performs supervisory visits as required.
• Works with RN supervisor and Branch Manager on planning and evaluating programs and services.
• Attends staff meetings and trainings; ensures regulatory compliance.
• Follows state/local/payer requirements; maintains proper documentation.
• Ensures safe handling of medications; transcribes orders as needed.
• Maintains furnishings and supplies to prevent the spread of disease; schedules health-related appointments.
• Communicates with supervisor and scheduler regarding hospitalizations and updates.
• Provides new employee training and re-certification as needed; ensures competency.
• May cover hours, weekends, and holidays on call; other duties as assigned.
