Job summary

Namaste Home Health and Hospice is seeking a flexible Registered Nurse to join our West Grand County team. You will plan, organize and direct hospice care with emphasis on community health education, delivering compassionate nursing in patients' homes and communities.

Responsibilities include ongoing assessments, care-plan development, medication administration, family education, and coordinating with the interdisciplinary team to ensure high-quality care and on-call coverage as needed.

Qualifications

• Licensed RN in the State of Colorado.
• Minimum of two years’ nursing experience, preferably in home health, hospice or public health.
• Current CPR certification.
• Valid driver's license with reliable vehicle.
• Ability to lift and assist patients; strong communication skills.

Responsibilities

• Conduct initial and ongoing assessments of patient and family needs.
• Develop and implement care plans with the interdisciplinary team.
• Administer medications and treatments as prescribed.
• Educate patients and families on self-care and disease management.
• Coordinate patient care as Case Manager and participate in on-call duties.
• Document care accurately and maintain charting.
• Supervise hospice aides when required.
