Upland Hills Health is seeking a part-time Certified Nursing Assistant for its Home Health Services. The role involves assisting patients under nurse supervision, providing palliative and supportive care, and coordinating care with assigned patients.

Experience with EMR is preferred; driving to patient locations is required with a valid Wisconsin license. Prior home care or hospice exposure is a plus.

Qualifications

• Must have completed a Nursing Assistant Program or its certified counterpart and have a valid nursing assistant certification with the Division of Health and be on the State and Federal Registries.
• Candidates must have a reliable car available during working hours or accessibility to dependable transportation.
• Must have a good driving record and active Wisconsin Driver’s License. Driving in an essential function of the job and must remain insurable.
• Prefer prior Nursing Assistant experience in an institution or in a home care/hospice setting.

Responsabilités

• The Home Health/Hospice Aide is a Certified Nursing Assistant that works under the supervision of a nurse performing selected duties that do not involve skilled nursing or require nursing judgment.
• These duties include palliative, supportive, supplementary and complementary to nursing practice.
• The Certified Nursing Assistant will see only patients assigned by the nurse coordinating the care of the patient.
• Home Health/Hospice Certified Nursing Assistants may see both Home Health and Hospice patients in their homes or area health care facilities as are needed.
