Qualifications

• Current Texas Certified Nurse Aide (CNA) certification in good standing.
• CPR/BLS certification preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personal care as directed by the patient’s plan of care, including bathing, grooming, dressing.
• Assist with mobility, transfers, positioning, and ambulation.
• Assist with activities of daily living (ADLs).
• Observe and report changes to the supervising nurse.
• Measure and document vital signs when assigned.
• Maintain accurate documentation of visits and services.
• Follow infection-control, safety, and patient-rights requirements.
• Maintain patient confidentiality and comply with HIPAA.
• Communicate professionally with patients, families, nurses, and care team.
• Travel to patient homes within the assigned service area.
