Md-Home-Health-Md-Home-Assis is seeking a Certified Nursing Assistant to provide direct personal care to patients in their homes under RN supervision. Experience in home health and reliable transportation are required.

You will work within an established agency with a long-standing presence in the valley. The role focuses on assisting with admissions, transfers, and discharges, and ongoing review of care plans to ensure patient needs are met according to care plans.

Qualifications

• Must have CNA experience in home health.
• Must have reliable transportation.
• Forty-year established home health agency in the valley.

Responsabilités

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan
