Responsibilities

• Assist with patients' personal care needs including hygiene, bathing, dressing, and ambulation per Plan of Care.
• Plan and prepare nutritious meals; document services promptly per policy and personal care plan.
• Maintain confidentiality at all times.
• Report changes or incidents to RN Supervisor.
• Carry out other duties.
• Accompany patients to doctor visits.
