Job summary

Always Compassionate Home Care in New York is seeking a dedicated Home Health Aide to support clients in their homes. You will follow a nurse's care plan, monitor conditions, assist with hygiene, meals, mobility, and daily activities.

Qualified candidates hold NYS PCA/HHA certification, are at least 18, have a high school diploma, and demonstrate excellent communication and compassionate care. We offer flexible hours, weekly pay, and health insurance for full-time staff.

Qualifications

• Must have NYS certification for PCA & HHA's
• Must be 18 years old
• High school diploma or equivalent
• Preferred six (6) months experience
• Meets health state requirements

Responsibilities

• Monitor patient condition by observing physical and mental conditions, intake and output and reporting changes to nursing supervisors.
• Maintain a healthful, safe environment.
• Provide nutritional and environmental support.
• Assist with personal hygiene, toileting, dressing, walking and feeding the client.
• Position the client as per care plan.
• Assist patients with personal services, such as bathing, dressing and grooming.
• Support patient with housekeeping and laundry services, food shopping and errands as per care plan.
• Prepare and serve meals and snacks as directed by care plan.
• Assist with treatments and therapy as ordered by physician and approved by nursing supervisor.
• Remind the client to take medications.
• Assist patient with shopping respecting budget and cost-saving techniques.
• Obtain patients' vitals, document values, and inform nursing supervisor as required.
• Utilize Electronic Visit Verification for daily attendance and timesheets.
• Maintain a safe environment by infection control protocols.
• Follow dietary requirements and nutrition standards per care plan.
• Use medical supplies and equipment correctly when appropriate.
• Provide emotional support and build trust with the client.
• Encourage client independence.
• Protect the agency by following professional standards and policies.
