Job summary

Ohio Living is seeking a dedicated Home Health Aide to provide high-quality personal care and support to patients in their homes. You will assist with bathing, dressing, mobility, meals, and medications while maintaining documentation in the EMR system.

A valid CNA and Medicare competency are required, with experience preferred. This role requires reading-English proficiency, a valid driver's license, and flexibility to work scheduled weekends/evenings as needed.

Qualifications

• High school diploma or equivalent required.
• CNA in good standing on the Ohio Department of Health Nurse Aide Registry required; or
• Successful completion of the Medicare competency evaluation program for home health aides.

Responsibilities

• Provides psychosocial support and personal care to the patient, including bathing, oral hygiene, hair care, nail care, and dressing.
• Assists patient with mobility and transfers using assistive devices.
• Assists with prescribed exercises and activities per care plan.
• Assists with toileting needs and incontinence care.
• Plans and prepares nutritious meals per diets; cleans kitchen area.
• Assists patients to self-administer medications with reminders.
• Performs homemaking tasks and accompanies patient to appointments per care plan.
• Observes and documents patient status in EMR; reports observations.

Education

High school diploma or equivalent

Certified Nursing Assistant (CNA) in good standing on the Ohio Department of Health Nurse Aide Registry
