Job summary

Qualifications

Responsibilities

• Provides psychosocial support and personal care to the patient, including bathing and hygiene.
• Assists patient with mobility, transfers, and positioning.
• Assists with prescribed exercises and toileting needs.
• Plans and assists with nutritious meals and prescribed diets.
• Assists patients with medication reminders and administration support.
• Performs homemaking tasks and accompanies patient to appointments as needed.
• Documents observations and care in the EMR per standards and regulations.
• Drives patients to appointments and other activities as required.
• Performs other duties as assigned.

Education
