Job summary

CenterWell Home Health is seeking a compassionate Home Health Aide to support patients at home under the supervision of an RN and according to their Plan of Care.

You will assist with daily activities, monitor vital signs, manage medications under guidance, and help patients maintain independence and quality of life in a home setting. Strong communication and adherence to infection control are essential.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver’s license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsibilities

• Provide direct patient care under the RN direction and according to the Aide Plan of Care.
• Assist with self-administered medications and monitor dosing.
• Take and report temperature, pulse and blood pressure.
• Maintain patient hygiene and assist with bathing, grooming and skin care.
• Assist with feeding and monitor nutrition and fluid intake.
• Assist with toileting needs and ostomy care when applicable.
• Support mobility, transfers, positioning and ambulation as per care plan.
• Document care provided and communicate changes to the team.
• Follow infection control practices and maintain a safe home environment.
• Assist with light household tasks if needed and allowed by plan of care.
