Job summary

CenterWell Home Health is hiring a Home Health Aide to deliver compassionate in-home care under RN supervision. You will support rehabilitation, assist with daily living activities, monitor vitals, and help patients regain independence in their own homes.

The role requires CNA/CHHA certification within the last 24 months, a valid driver’s license and reliable transportation, and at least one year of relevant experience in a healthcare setting. Benefits include paid time off and retirement plans.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency in the last 24 months.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist with self-administered medications and observe dose accuracy.
• Take vital signs and report abnormal results to supervisor.
• Assist with personal hygiene and activities of daily living.
• Assist with feeding, nutrition education, and meal setup.
• Assist with toileting and catheter/ostomy care as required.
• Support patient mobility, exercises, transfers, and ambulation per Plan of Care.
• Document care interactions clearly and timely for continuity of care.
• Maintain infection control standards and provide a safe environment.
• Assist with light household tasks when needed and allowed.
