Job summary

CenterWell Home Health is hiring an Home Health Aide to support patients in the North Palm Beach County area. You will work under the RN, following the Plan of Care to improve function and independence in the home setting.

The role requires a recent CNA/HHA program completion, a valid driver’s license, and clean HHA registry status. At least one year of related experience is preferred, with competitive pay and benefits.

Qualifications

• Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) program completed within the last 24 months.
• Must meet applicable state certification requirements.
• Valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry and completed HHA/CNA course to work for a Medicare-certified agency.
• Minimum one year of Home Health Aide or CNA experience in past 24 months.

Responsibilities

• Provide direct patient care under supervision of the RN and according to the POC.
• Assist with medications by opening bottles, reading labels, and ensuring correct dosing.
• Take vital signs and report abnormal results to supervisor.
• Support personal hygiene and daily living activities.
• Assist with feeding and monitor nutrition and fluid intake.
• Assist with toileting and catheter care as needed.
• Support mobility, positioning, transfers and ambulation per plan of care.
• Document care interactions clearly and timely.
• Maintain infection control and a clean, safe home environment.
• Assist with light housework and shopping if needed and allowed.
