Job summary

TheraCare Home Health in Livingston, TX is seeking a reliable Home Health Aide (HHA) to deliver compassionate in-home care under RN supervision. You will assist with activities of daily living, ensure safety, document care, and communicate with patients' families and the clinical team to maintain continuity of care.

The position is PRN with duties focusing on elderly and ill patients, requiring a Certified HHA program and CPR certification, and a commitment to quality home health care.

Qualifications

• Completion of a Certified Home Health Aide (HHA) Program.
• Minimum of 1 year of experience as a Home Health Aide in a home health agency.
• Ability to follow an established Plan of Care under RN supervision.
• Excellent communication skills with patients, families, and clinical staff.
• Reliable transportation with valid driver’s license and current auto liability insurance.
• Current CPR certification.

Responsibilities

• Provide direct patient care under the supervision of a Registered Nurse in accordance with the Aide Plan of Care.
• Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing and toileting.
• Support safe patient transfers, ambulation, and positioning.
• Monitor and report changes in patient condition to the supervising nurse.
• Maintain a clean, safe, and comfortable home environment for patients.
• Complete accurate and timely documentation in the medical record.
• Communicate effectively with patients, families, and interdisciplinary team members.
