Job summary

Entrusted Hearts seeks a dedicated Home Health Aide to assist clients with activities of daily living under the supervision of a registered nurse. You will help with personal care, mobility, vital sign recording, and meal preparation while promoting a safe, clean environment.

The role requires a High School diploma or equivalent, Home Health Aide certification, and BLS certification, with 6–12 months of relevant experience. Ability to stand long periods and lift is expected.

Qualifications

• High school diploma or equivalent is preferred.
• Home Health Aide Certification and BLS certification are required.
• 6–12 months of experience in a client care facility with basic skills and medical terminology.

Responsibilities

• Performs indirect client care activities.
• Insures safety during client care.
• Observation, reporting and documentation of client status/services furnished.
• Demonstrates efficiency, effectiveness, and dependability in providing client care.
• Maintains effective communication with clients, representatives, caregivers and HHA team members.
• Basic infection prevention and control procedures.
• Safe transfer techniques and ambulation.
• Appropriate and safe personal hygiene and grooming assistance.
• Participates in QAPI program as requested.
• Supports and enhances Entrusted Hearts Home Care’s image by demonstrating a positive attitude in client interactions.
