Job summary

Granite Peak Home Health is seeking a Home Health Aide to deliver compassionate personal care in patients' homes under supervision of RNs and therapists. You will follow agency policies, document visits, and report changes in patient conditions to the RN.

The role requires CPR certification, a sympathetic attitude toward the sick and elderly, and the ability to read, write, and follow directions. Training and certification support are provided as needed.

Qualifications

• Meet Medicare conditions of participation in the Home Health Aide training program and be competent.
• If currently a licensed CNA, Granite Peak Home Health can provide Home Health Certification
• Sympathetic attitude toward the care of the sick and elderly.
• Ability to carry out directions, read and write.
• Maturity and ability to deal effectively with the demands of the job.
• Must have valid CPR certification

Responsibilities

• Understands and adheres to established Agency policies and procedures.
• Performs personal care, bath, and hands-on care as assigned.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Reports changes in the patient's condition and needs to the RN.
• Performs household services essential to health care in the home as assigned.
• Ambulates and exercises the patient as assigned.
• Performs simple procedures as an extension of the therapy or nursing services, e.g., range of motion (ROM) exercises as assigned.
• Assists with medications that are ordinarily self-administered as assigned.
• Attends in-service and continuing education programs as scheduled and necessary.
• Attends patient care conferences as scheduled.
