Job summary

ANC Health at Home in Indiana is seeking a Home Health Aide to deliver compassionate in-home care, assist with daily living activities, mobility, and documentation.

You will work within our health care network, follow established care plans, coordinate with families and healthcare professionals, and contribute to personalized care strategies for patients in their homes.

Qualifications

• High School Diploma or equivalent is required.
• CPR/BLS certification is required through American Heart Association or American Red Cross.

Responsibilities

• Assist clients with daily living activities (bathing, grooming, dressing, feeding).
• Assist patients with mobility and transfers.
• Complete documentation accurately and promptly.
• Communicate with clients, families, and health professionals regarding needs and concerns.
• Follow care plans and contribute to personalized care strategies.

• Assisting patients with mobility and transfers.

• Following established care plans and contribute to the development of personalized care strategies.

Job Requirements

• High School Diploma or equivalent is required.
• Successful completion of a competency evaluation program.
• Candidates must be listed on the state Nurse Aide Registry
• One year home health aide or nurse aide experience
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements
