CommonSpirit Health at Home is seeking a Home Health Aide to support patients in Lexington, KY. This PRN role focuses on daily living assistance, mobility support, and accurate documentation to ensure high-quality at-home care.

The ideal candidate has a high school diploma, a state CNA/Nurse Aide registry listing, completed competency evaluation, and CPR/BLS certification, with at least one year of home health or nurse aide experience.

Qualifications

• High School Diploma or equivalent required.
• Competency evaluation completed.
• Listed on the state Nurse Aide Registry.
• At least 1 year of home health or nurse aide experience.
• CPR/BLS certification required.

Responsabilités

• Assist clients with daily living activities (bathing, grooming, dressing, feeding).
• Assist patients with mobility and transfers.
• Complete documentation accurately and promptly.
• Communicate with clients, families and healthcare professionals about needs.
• Follow care plans and contribute to personalized care strategies.

*Pay Range*
