Hackensack Meridian Health at Home is hiring for in-home care teammates to support patients with personal care under RN guidance. You will assist with bathing, grooming, mobility and daily living activities in a home setting, following the individual care plan.

The role requires good customer service, a valid driver’s license, and Homemaker - HHA certification, with travel reimbursement and a comprehensive benefits package as part of Total Rewards.

Qualifications

• Good customer service skills are required.
• Education, knowledge, skills and abilities preferred include home care experience.
• Licenses and certifications include Homemaker - HHA Certification and a valid driver’s license.

Responsabilités

• Provide in-home, para-professional services for patients under RN direction.
• Assist patients with personal care activities and maintain safety per care plan.
• Record and document patient information accurately in charts.
• Assist with transfers, ambulation and range-of-motion exercises.
• Perform light housekeeping and meal-related tasks.
• Support patient self-administration of medications and follow dietary instructions.
• Ensure proper hygiene for ostomy care and catheter care as needed.
• Demonstrate flexibility and adhere to organizational competencies.
