Job summary

Care 365 Homecare seeks a Personal Care Aide to assist patients in home or facility settings under the supervision of a registered nurse. Duties include nutrition, environmental support, personal hygiene, and basic medical tasks as directed.

The role requires NYS DOH approved training or equivalency, compliance with inservice education, and being at least 18 years old. Locations include Yonkers and surrounding areas across Westchester County.

Qualifications

• Must hold a NYS DOH approved training in home health aide services or an equivalency/competency program.
• Compliance with Inservice Education requirements.
• Minimum of 18 years of age.

Responsibilities

• Provides personal care and rehabilitation services and assists in activities of daily living as directed by the nurse or therapist.
• Follows care plan instructions as directed by the nurse.
• Provides Environmental Support including light housekeeping and meal preparation.
• Assists with feeding, nutrition tracking and personal hygiene.
• Maintains daily patient records and timesheets and reports changes to the nurse.

NYS DOH approved Basic Training Program in home health aide services

NYS DOH approved Equivalency Examination or Competency Only Program in home health aide services

NYS DOH/Department of Social Services approved Training or Competency Program in personal care services
