Job summary

Care 365 Homecare is seeking a compassionate Home Health Aide to provide daily living and personal care support in patients' homes under an established care plan. You will assist with personal care, monitor vital signs, and support mobility, with tasks ranging from light housekeeping to errands.

Responsibilities include following care plans, transporting clients as needed, and maintaining confidentiality. A high school diploma and a caring, empathetic attitude are essential.

Qualifications

• Must be 18 years or older.
• Must pass a criminal background check.
• Good communication and interpersonal skills.
• Ability to follow written and verbal instructions.
• Ability to work with limited supervision.
• Caring, compassionate and empathetic.

Responsibilities

• Provide health care services in patients' residences.
• Perform domestic and household tasks.
• Transport and accompany patients to doctors' appointments or hospital.
• Assist clients with personal care activities.
• Monitor patients' vital signs and report conditions.
• Assist with mobility and physical therapies/exercises.
• Provide companionship and basic emotional support.
• Follow care plans and report completed tasks.
• Light housekeeping, meal preparation & laundry.
• Maintain patient confidentiality and attend as required.
