Job summary

Care 365 Homecare seeks a compassionate Home Health Aide to provide daily living assistance and personal care to patients in their homes under an established care plan. You will assist with mobility, personal care, meal prep, light housekeeping, and essential errands while monitoring health and reporting changes to the care team.

The role requires empathy, strong communication, reliability, and the ability to follow written and verbal instructions with minimal supervision.

Qualifications

• Must be 18 or older.
• Must be able to pass a criminal background check.
• Excellent communication and interpersonal skills.
• Ability to follow written and verbal instructions.
• Ability to work with limited supervision.
• Caring, compassionate and empathetic with a genuine desire to care for others.
• Ability to monitor vital signs.
• Familiarity with basic nutrition and personal hygiene standards.
• Caring and compassionate personality.
• High school degree or equivalent.

Responsibilities

• Provide health care services in patients' residences.
• Perform domestic and household tasks.
• Transport and accompany patients to doctors' appointments or hospital.
• Assist with clients' personal care activities.
• Monitor patients' vital signs and report on condition.
• Assist with mobility and physical therapies/exercises.
• Provide companionship and basic emotional support.
• Follow care plans and report completed tasks.
• Assist with personal care needs (bathing, dressing, etc.).
• Provide light housekeeping, meal prep, laundry, and essential errands.
• Maintain patient confidentiality at all times.
• Maintain regular and flexible attendance to meet client needs.
• Abide by all regulations and policies.
