PurposeCare is seeking compassionate Home Health Aides/CNAs to provide one-on-one in-home care. You will assist with personal care, mobility, meals, meds reminders, and essential documentation across assigned visits within the service area.

Ideal candidates are 18+, have or will obtain HHA/CNA certification, and can travel to client homes with reliable transportation. Training includes dementia care and safety protocols.

Qualifications

• Active HHA or CNA certification or willingness to obtain certification
• Ability to follow written care instructions and document services
• Compassion, patience, and professionalism with vulnerable populations
• Ability to work independently in home settings
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements

Responsabilités

• Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders (no independent med administration)
• Monitor and report patient condition and changes to the supervising nurse
• Take and record basic vital signs when trained and assigned
• Maintain a clean, safe, and orderly client environment
• Assist with linens and light client laundry as appropriate
• Use safety equipment properly (rails, supports, stools, etc.)
• Document care provided according to company and regulatory standards
• Travel reliably to patient homes and assigned visits across your service area
• Complete required in-service training, including annual dementia education
