Job summary

UNIFI Healthcare is seeking a Home Health Aide to provide essential personal care and supportive services under supervision. You will assist clients with ADLs, promote safety and independence, and document care per the Plan of Care.

The role emphasizes compassionate, patient-centered support, with ongoing training and RN supervision. Flexible scheduling and a commitment to home-based care are offered.

Qualifications

• Completion of an approved HHA training program.
• Current CPR/First Aid certification.
• TB screening within the past year.
• Clear state background check.
• Reliable transportation with valid auto insurance.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Assist clients with ADLs including bathing, dressing, grooming, toileting, transfers and meal support.
• Provide companionship, escort to appointments, and assist with errands as needed.
• Document care, vital signs, intake/output, and concerns the same day.
• Follow Plan of Care, infection control, and safety protocols; report changes to RN Case Manager.
