Job summary

Unifi Healthcare is seeking a Home Health Aide (HHA) to provide essential personal care under RN supervision in a home setting. You will work closely with clients and families to promote safety, comfort, and independence while following the Plan of Care.

The role includes ADL assistance, documentation, safety compliance, and ongoing training, with flexible scheduling opportunities and a supportive team culture.

Qualifications

• High school diploma or GED.
• Completed an approved HHA training program.
• Current CPR/First Aid certification.
• TB screening within the past year.
• Clear state background check.
• Reliable transportation with valid auto insurance.
• Ability to lift at least 25 pounds.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
• Support transfers and ambulation; assist with turning and repositioning to prevent pressure injuries.
• Provide meal planning, preparation, and feeding support per Plan of Care.
• Document care and vital signs; report changes to RN Case Manager.
• Perform light housekeeping, accompany clients to appointments, and provide companionship.
• Engage clients in recreational activities and social interactions; offer emotional support.
