Job summary

UNIFI Healthcare is seeking a Home Health Aide (HHA) to provide essential personal care under the supervision of the RN Case Manager or designated administrator. You will work closely with clients and families to promote safety, comfort, and independence, following the Plan of Care and documenting care accurately.

Responsibilities include assisting with ADLs, meal support, transportation, and engaging clients in activities.

Qualifications

• High school diploma or GED is required.
• Completion of an approved HHA training program.
• Current CPR/First Aid certification.
• TB screening within the past year.
• Clear state background check.
• Reliable transportation with valid auto insurance.
• Ability to lift at least 25 pounds.
• Ability to read, write, and communicate effectively in English.
• Strong communication skills and ability to follow written and verbal instructions.

Responsibilities

• Assist clients with ADLs (bathing, dressing, toileting, transfers, turning and repositioning).
• Provide meal planning, preparation, and feeding support per Plan of Care.
• Empty catheter/colostomy bags and document outputs.
• Light housekeeping and transportation to appointments.
• Engage clients in recreational activities and provide companionship.
• Document care, vital signs, intake and output; report changes promptly.
• Follow Plan of Care; maintain infection control; report emergencies.
