Qualifications

• Active HHA or CNA certification or willingness to obtain certification
• Age 18 or older
• Willingness to complete competency and skills assessments
• Ability to follow written care instructions and document services
• Reliable transportation and ability to travel to client homes
• Ability to lift up to 25 pounds
• Willingness to complete TB testing, background screening, and onboarding requirements
• CPR and Adult First Aid certification would be a plus

Responsibilities

• Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting
• Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan
• Prepare meals and assist with feeding as needed
• Support medication self-administration through reminders
• Monitor and report client condition to supervising nurse
• Take and record basic vital signs when trained
• Maintain a clean, safe home environment
• Travel to patient homes and assigned visits across service area
• Complete required in-service training including dementia education
• Document care provided according to standards
