Job summary

Life Care Home Health Family is hiring a Home Health Aide to support clients in their residences across Texas. The role requires compassion, reliability, and the ability to follow a personalized care plan, with duties including transferring, mobility assistance, feeding, and basic health maintenance.

The ideal candidate has at least 1 year of HHA/CNA experience, current CPR certification, a valid driver’s license, and the ability to communicate clearly in English.

Qualifications

• 1 year experience as a home health aide or CNA in a home health setting.
• Formal training in nursing arts may substitute for the qualifying experience, subject to applicable State law/regulation.
• Current CPR certification.
• Current driver’s license in good standing; mobility required for client visits.
• Home Health Aide Certificate or other similar certification held in good standing.

Responsibilities

• Assist clients with tasks within the Plan of Care (POC).
• Communicate with administrative staff regarding client needs and changes.
• Provide care and document services per the Home Health Aide Care Plan.
• Participate in team meetings and maintain continuity of care.
• Apply infection control and universal precautions in patient care.
