Job summary

Pinnacle Home Care in Florida’s Inverness area is seeking a PRN Home Health Aide (HHA) to support in-home patient care across Citrus County and surrounding areas.

Eligible candidates hold a CNA or HHA license and have completed at least 75 hours of training, plus a valid driver’s license for home visits. This role supports independence and dignity for patients in their own homes.

Qualifications

• Must have CNA or HHA license and at least 75 hours of training.
• Must have valid driver’s license and access to reliable vehicle.
• Ability to follow care plans and maintain patient dignity.
• Excellent communication and documentation skills.
• Ability to lift and assist patients and visit homes.

Responsibilities

• Adhere to care plan to ensure safe and competent care for the patient.
• Assist patients with personal hygiene and support the maintenance of a clean, safe, and healthy environment.
• Help patients with ambulation as prescribed by the physician and approved or supervised by a clinician or therapist.
• Support therapy personnel as needed during rehabilitative processes.
• Encourage patients to achieve maximum independence according to the nursing care plan.
• Promote mental alertness by engaging patients in activities of interest.
• Provide emotional and psychological support to patients and household members, fostering trust and confidentiality.
• Report any changes in the patient’s condition or home environment to the RN or clinical manager.
• Perform light housekeeping tasks related to creating a safe and comfortable environment, as directed by the clinician.
• Complete visit reports promptly and accurately within the electronic medical record system.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agency’s process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
• Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
