Job summary

VitalCaring Group is seeking a Certified Nursing Aide (CNA) to perform home health care tasks in patients' homes, providing compassionate, hands-on support. You will assist with daily living activities, mobility, and vital signs while documenting care and coordinating with the clinical team.

This PRN role offers flexible, patient-centered work in a home-based setting. Ideal candidates meet state/federal Home Health Aide requirements, are 18+, CPR certified, and have a valid driver’s license with

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions

Responsibilities

• Provide compassionate, hands-on care that supports patient comfort, safety, and independence
• Assist patients with daily living activities (bathing, dressing, grooming, toileting)
• Support mobility, transfers, and basic exercises per care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding
• Take and record vital signs as trained
• Assist with medication self-administration as directed
• Observe and report changes to the clinical team
• Document care provided accurately in the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit
