Job summary

Allina Health in Minneapolis, MN seeks a Home Health Aide to provide routine personal care to patients in home health and hospice settings. This role includes comprehensive training and an experienced team, with a day shift and occasional weekend rotation.

You will support patients with daily living activities and essential care needs. The position offers a stable, full-time schedule (1.0 FTE), with 80 hours per two-week pay period, and a pay range aligned to local wages.

Qualifications

• Must be 18 years of age with education and/or experience needed to meet required functional competencies as listed on the job description
• 0 to 2 years as a Nursing Assistant or Home Health Aide
• Preferred: High school diploma or GED
• Preferred: 2 to 5 years as a Nursing Assistant or Home Health Aide

Responsibilities

• Provides safe, competent personal cares to patients in Home Health, Hospice and Residential Hospice settings.
• Monitors and reports changes in patient health status to Case Manager.
• Assists with basic residential household activities.
• Documents all patient visits and care provided; communicates changes to supervising clinician.
• Completes agency, state and federal regulatory requirements for continuing education.
• Other duties as assigned.
